Purpose of the study
The presentation of an interesting case with an anatomical variant in which the hip joint capsule communicates with the bursa of the iliopsoas.
Material and method
A man of 35 presented with pain in the right hip that had been worsening for two months. The patient brought an MRI whose report described a fluid collection within the iliopsoas muscle. Bone oedema was also found in the anterior acetabulum, indicative of femoroacetabular impingement. The patient underwent hip arthroscopy, during which a communication was found between the joint capsule and the bursa of the iliopsoas. A tear of the labrum and a femoroacetabular impingement lesion were also found, and were treated accordingly.
Conclusions
Communication between the hip joint capsule and the iliopsoas bursa may be present in a small proportion of patients. Interpretation of the MRI requires care, and it does not constitute an indication for hip arthroscopy.
