Purpose of the study
The presentation of the case of a 50-year-old patient with chronic pain in the hip joint, without radiological findings of osteoarthritis, who was managed arthroscopically.
Material and method
Plain radiography of the hip was negative for osteoarthritis (normal joint space, no subchondral bone sclerosis, no cysts), while it did show features of pincer-type femoroacetabular impingement. MRI was negative for necrosis of the femoral head or transient osteoporosis, while it was positive for degeneration of the acetabular labrum. Hip arthroscopy was performed, during which the degeneration and ossification of the labrum were confirmed. Treatment consisted of smoothing it and recession of the ossified portion (acetabular recession). The articular cartilage of the acetabulum and of the femoral head was normal.
Results supporting the conclusions
The patient was reviewed at 6 weeks and 3 months postoperatively. A considerable reduction in pain was observed and she is following the postoperative rehabilitation protocol normally. A considerable improvement was also observed in the Harris and Non-arthritic Hip Score hip rating scales.
Conclusions
Hip pain, even in the 5th decade of life, is not always due to osteoarthritis. Arthroscopic treatment of degeneration and ossification of the acetabular labrum offers the patient relief from the symptoms.
